The Illinois 5Essentials Survey is administered annually by the University of Chicago and facilitated locally by schools via an online portal. All survey responses will be kept confidential and will only be reported as part of the comprehensive school report, not individually. The participation rate goal is 95% for all stakeholders.
Links to the survey became active on Tuesday, February 18, 2025. The survey will take approximately 10 minutes to complete. To take the brief survey, please follow these steps:
Parents/Guardians:
- Visit http://survey.5-essentials.org/illinois/survey/parent/
- Select language preference
- Type your email address
- Type and select “Cook” as “County”
- Type “DD Eisenhower High Sch” into the “School” field, select the school, then log in.
Answer all applicable questions and click submit survey.
