1.       The Parent/Guardian must provide ONE of the following proofs of identification:

Current Illinois Driver's License or Illinois State Identification Card showing In-district address

OR

Other Photo Identification and a voided personal check stub or credit card statement with a valid address

2.      The Parent/Guardian must ALSO provide the following proof of residency:

·         Two Current Utility Bills (Last 2 months) Phone, Water, Electric or Gas. Copies of the utility envelopes are not acceptable.

·         Proof of Automobile/ Vehicle Insurance (Must have an address on document)  

·         Proof of Vehicle Registration

·         Current Public Aid Card (if applicable)

3.       The Parent/Guardian must ALSO provide ONE of the following along with the utility bills:

Proof of Mortgage or Lease (Lease must include property owner name and contact phone number)

OR

Proof of Property Tax

4.       Items Needed From the Previous School

·         ISBE (Illinois State Board of Education) transfer form

·         Unofficial transcripts

·         Withdrawal grades (if applicable)

·         Test scores

·         IEP or 504 Plan (if applicable)

5.       In Addition:

·         Copy of the student’s birth certificate

·         9th grade physical with immunizations

·         Dental record (freshmen only)

·         Appropriate legal documentation of custodial care (if applicable)
